After having surgery to unblock arteries, Dan found the knowledge and encouragement to take control of his life and get healthy.
Dan S. led an active life and was in pretty good physical shape. One day while out for a jog, he noticed that his breathing was more labored than it should have been. Concerned, he went to see his doctor. After undergoing a series of tests, Dan learned that two of his coronary arteries were blocked. His doctor assured him that although his heart was fine, without surgery to correct the arteries, Dan could be in serious trouble.
The day after he received this news, Dan had the corrective surgery. It was while he was at home recovering, that he received a phone call from a nurse, inviting him to join the ConditionCare program. "I'd just gotten the scare of my life, so I was ready to listen," says Dan of his decision to join the well-being program that is offered to qualifying members at no additional charge.
Once enrolled in the program, Dan began receiving phone calls from Laurie Lande, RN BSN, his ConditionCare Nurse Care Manager. "Laurie was wonderful and very knowledgeable," says Dan of their bi-monthly phone consultations. "She was an invaluable combination of coach and trusted family member along with health advisor and dedicated supporter."
During their calls, Dan and Laurie discussed how he was doing, upcoming procedures, and ways that he could work with his doctors to improve his health. "Laurie was the spark plug that kept me on track," says Dan of their conversations. "I made dramatic changes in my diet, took my medication as directed, started a walking regimen and began to have my cholesterol level checked on a regular basis. I knew I wanted to be around for my family."
Dan's work with Laurie went so well that he is now a ConditionCare graduate. "A lot of people don't realize that they can take control of their health by making some lifestyle changes," says Laurie. "Dan became very proactive with his health through diet and exercise. He's now feeling better, eating healthier, and exercising more." Dan agrees, noting that since joining ConditionCare, he has lost nearly 20 pounds and has added weight lifting to his exercise routine.
"ConditonCare is a great program," says Dan. "When you're diagnosed with a major illness, it shakes you to the core. You need someone who knows what they're talking about to tweak you now and then to help you stay on the right path to recovery."
